Not sure about how loud the noise is,but my Johnson 115 used to make a little clunking noise when shifting in and out of gear. I changed the lower unit oil to Amsoil synthetic oil and it has eliminated the clunking noise.
IF the noise is deafing you may have some worn gears in the lower unit and that could be a major repair. Have you checked the lower unit oil? Is it full? Does it have a milky look ? Does have a metalic look or a grit feel to it?
If it has either I would change the oil to a synthetic oil like Amsoil and try that. Be sure to get the right weight oil for your outboard. If it still makes a loud clunking noise then you may have some badly worn lower unit gears.
Hope this helps get you started in resolving the issue.
